Overview

Project ECHO (Extension for Community Healthcare Outcomes) is a collaborative model of medical education and care management that seeks to empowers clinicians everywhere to provide better care to more people, right where they live. The focus of this ECHO project is to provide the enhanced diabetes knowledge to clinicians that will allow them to improve diabetes care to patients in underserved areas by viewing on-demand learning from recorded sessions.  The format is a series of 16 on-demand sessions recorded from virtual telementoring conferences, each about 1 hour. Each session includes about 25-40 minutes didactic presentation(s) and 25-40 minutes with community provider case discussion and Q&A.

Registration
All participants will be asked to pay a one-time fee of $30 to access the modules below. This fee covers access to the online content for the entire 16 modules listed below. Once you have paid and registered, you will receive an email when your payment is processed with instructions on how to access the content and provided a PROMO code.  Please reference that attachment on your receipt.  Registration for each individual module is also available for $25 each.

METHOD OF PARTICIPATION
In order to complete this activity, access to the internet and a device is required. You must receive a minimum passing score of 80% on each module post-test (max. 2 attempts) in order to receive your CME credit. Each module will have a required pre-test, followed by the educational content, a post-test and evaluation. It is important to know that once you have started the Post-Test (Examination), if you exit the Post-Test without responding to all questions, it will be counted as a failed attempt. Only two (2) attempts are permitted.  To access the content, select a session from the list above.  After you have reviewed the content information, select the tests tab to be taken to the content.

INTENDED AUDIENCE
This educational program is intended for physicians, advanced practice providers, nurses, pharmacists, residents and fellows in primary care.

OBJECTIVES Upon completion of this activity, attendees should be able to:

  • Use evidence-based strategies to improve overall management of type 2 diabetes.
  • Discuss and review new diabetes medications and use evidence-base strategies for clinical indications for use.
  • Enhance the use of multi-disciplinary approaches to diabetes management through case based learning.
