The Office of Continuing Medical Education reserves the right to cancel this course prior to the course date when a minimum enrollment is not met or for other reasons that prevent the University from holding the course. IMPORTANT PAYMENT INFORMATION

Per UW CME policy, you are not considered registered until full payment is received. Unpaid orders do not reserve spots for conferences. If you have an unpaid order when registration rates increase, your order will automatically increase to the higher late registration rate.

REFUND & CANCELLATION POLICY

The Office of Continuing Medical Education must receive written notification of your cancellation by February 26, 2025. A $75 processing fee will be deducted from refund requests received by this date. No refunds will be issued after February 26, 2025. • Attendees must stay home if they have COVID-19 symptoms, have tested positive in the 5 days leading up to the event, or are still isolating after testing positive.
• COVID-19 testing within 72 hours of the event is recommended if using a PCR test, testing the day of the event is recommended if using a rapid antigen test. Testing 3 to 5 days after attending an event is also encouraged.
