DS2438: Cardiometabolic ECHO Program 2024-2025

The Cardiometabolic ECHO Project Replication project is a Washington/PNW region ECHO with the goals to enhance complex cardiometabolic management and reduce therapeutic inertia and racial inertial/disparities for medication access with focus on renal complications. Project ECHO is a telementoring model that enhances knowledge and expertise for primary care physicians to treat chronic diseases. The prevalence of diabetes and other cardiometabolic diseases is increasing and, despite newer medications and advancing technology, patients continue to have increased risk for catastrophic cardiovascular and renal outcomes – disparities are amplified in minority populations. Underutilization is likely in part due to lack of primary care providers’ comfort level using these medications. As well more limited knowledge on lifestyle counseling is needed. Our goal is to support the primary care providers at the front lines of complex cardiometabolic care through this ECHO project.

1. • Use evidence-based strategies to improve overall management of type 2 diabetes with focus on renal complications, lipids, obesity and cardiometabolic disease.
2. • Understand new diabetes, renal , obesity, and cardiac medications and use evidence-based strategies for clinical indications for use.
3. • Enhance the use of multi-disciplinary approaches to diabetes, renal complications and obesity and management through case-based learning.
