Overview
Dementia ECHO aims to improve the skills of providers around the diagnosis, treatment and support of people with memory loss and dementia via a twice-monthly telehealth clinic. Dementia ECHO is attended by a multidisciplinary panel of experts, and medical providers whose patients reside in Washington State. At each session a member of the panel, or a guest presenter will deliver a 15-20 minute didactic. For the next 45 minutes to an hour, the group will review cases presented in advance by the learners. The didactics presented at each bi-monthly session will align with Best Practices around Diagnosis, Ongoing Care Support/Management, Advance Care Planning and Palliative Care, Need for Increased Support and/or Higher Levels of Care, Preparing for Potential Hospitalization and Screening for Delirium; these best practices are outlined in the BREE Report. The primary purpose of Dementia ECHO is to increase knowledge and understanding of person-centered quality dementia care in the following areas: 1. Early Detection, Risk Reduction and Prevention Approaches for Providers 2. Person-Centered Assessment and Care Planning 3. Co-Morbidities, Medical Management and Deprescribing for Providers 4. Skills, Education and Support Needs of Persons Living with Dementia and Caregivers 5. Evidence-Based Non-pharmacological Practices, De-escalation strategies and Caregiver Uplift Practices 6. Progressive Support for Activities of Daily Living 7. Building, Scaling and Supporting the Workforce 8. Growing Supportive and Therapeutic Environments 9. Interventions for Transitions in Care and Advance Care Planning 10. Evaluating Person-Centered Practices Dementia Echo will specifically focus on underserved and regional locations including the aging homeless.
